We want to offer care and support that:
- is co-produced with the people who use it
- reduces inequality and social isolation
- builds on individual and community strengths
- empowers people to lead independent lives, exercise choice and control, and be fully informed.
We will ensure that services are focused on meeting complex needs, as well as intervening early to prevent needs increasing and enabling everyone to be as independent as possible, for as long as possible.
