Self-assessment Online

Before we can arrange services for you, you need to have an assessment. An assessment is carried out to identify your needs and to decide whether you qualify for a service under the guidelines.

You can now use the Internet to start the assessment process. This is called self-assessment. Self-assessment gives people the choice to assess their own needs or carry out an assessment on behalf of someone else.

This assessment is for adults who are finding it difficult to manage every day activities. For example, you or someone you know may be having difficulty managing steps or stairs, getting in and out of bed or some aspects of personal care. You will be asked questions to help us understand your needs and what, if any, equipment or services we can provide to help you.

Who can use this service?
A self-assessment can be done by anyone aged 18 or over, living within Islington Council's boundary who is having difficulty carrying out everyday activities. Carers, friends or family can also complete the assessment on behalf of someone else living in Islington. If this is the case, we will always contact the person concerned to check that they wish to have the services we may offer.

Can I just ask for an assessment by Islington Council?

Yes, anyone who believes that they may have social care needs which may fall within the definition of community care services can ask for an assessment. Local Authorities with Social Services responsibilities have a duty to carry out a community care assessment when requested.

If I complete the self-assessment, am I automatically entitled to services?

No, people with needs that fall outside the Adult Social Care Services eligibility criteria do not have any right to receive social care support from us. You can read about the eligibility criteria here: Needs Assessment Islington Website

You are entitled to social care services only if we agree that you qualify for support. In some cases, if you qualify for support but have higher needs we will ask you to have a full assessment before services are provided.

What happens once I have completed my online self-assessment?

It will be automatically sent to our Information and Access team. You will receive a response informing if you, or the person you are completing the assessment for, are eligible for services. If your needs are complex and we think you require a more in-depth assessment, we will tell you this and arrange a home visit to you.

If we agree that you qualify for support, you will be able to choose from a range of equipment or services matching your assessed need. If you would prefer, we can arrange for you to have the cash equivalent for some of these services, so that you can arrange them yourself. This is called a Direct Payment

I understand that in order for Islington Adult Social Services to arrange the right services, the information I provide in the self-assessment may be shared with other social and health care professionals, who need to be involved in helping meet my needs.
