Self Assessment Online - This assessment is for adults with a physical illness or disablity, who are finding it difficult to manage every day activities. Before we can arrange services for you, you need to have an assessment. An assessment is carried out to identify your needs and to decide whether you qualify for a service under the guidelines.
You can now start the assessment process here online by completing the form - click on the link below. Online self-assessment gives people the choice to start the assessment process for themselves in their own time or for a relative or carer to complete the form on their behalf.
Linkline Alarm - You can also apply for a Linkline residential alarm. The Linkline service provides a 24 hour, 365 day a year community alarm service to over 1,600 Islington residents. If you are elderly or vulnerable, have a physical disability or special needs - regardless of your age - this service could be for you.
