• adults with physical or sensory disabilities
• adults in chronic ill-health

at the Whittington Hospital, based in Archway.

Going into Hospital

For most people, going into hospital can be a worrying experience. Apart from the illness itself, you may also have to cope with other practical or emotional problems.

Islington Social Services has social workers and care managers at the Whittington Hospital. If you need help, we will see you to:

• carry out a community care assessment
• help you to plan your care on leaving hospital
• contact the social services from your home borough, if that is not Islington
  
  

Who we Help

Hospital social workers work with:

• older people
• people with physical or sensory disabilities
• people in chronic ill-health
• all who are unable to leave hospital and return to independent living

Services Provided

Most wards in the Whittington Hospital have a named social worker who will see all the patients who need help before they are discharged from hospital

If you are eligible, hospital social workers will work closely with you and your carers and with the medical, nursing and therapy teams to look at what help you will need when you leave hospital. Such help may include:

• assessment for community care services, including meals in the home, personal care, domestic support and day care
• assessment for and advice about choosing a residential or nursing home or extra care sheltered housing, if that is what you need
• advice on the provision of alternative housing
• information about services provided by other relevant organisations or departments that may be able to help you
• advice on agencies which can assist with financial problems, including welfare benefits
• help with some of the practical issues involved in hospital admission, such as the care of pets

Paying for our services

Some of our services are free.

Some of our services have a set charge, and some of our services have a sliding scale of charges, depending on how much money you have.

There is never a charge for a community care assessment.

We will always give you information about any charges, so that you know how much the services will cost before you receive them.
