EYIT has produced a number of different forms which early years SENCOs may find useful for developing their Special Educational Needs (SEN) provision and recording practice and provision for each child. These are suggested formats that many Islington SENCOs use, but you may also adapt them for the needs of your own setting. Please note: All documents can be downloaded from the bottom of the page.
Monitoring Form
This form comes with guidance for use and provides an opportunity to record the monitoring stage. This is when concerns have been raised about a child and information is being gathered about levels of development and progress made through observation and discussion with parents and staff.
Observing Children’s Behaviour Form
This form can be used to record the behaviour of individual children. It enables practitioners to look for patterns in behaviour that may help identify causes and triggers.
EYIT Referral Form
If SENCOs or managers are concerned about a child’s development they can refer that child to the EYIT team (with the parents’ agreement). An allocated member of the team will then work in partnership with setting staff and the parents, to plan and review provision to support the child’s progress.
Vision Impairment Support Referral Form
If SENCOs or managers want to develop their provision to include a child with vision impairment they can use this form to make a referral (with parents' agreement). A member of the EYIT team with RNIB training will work in partnership with practitioners, to plan and review provision to support the child’s progress. Download a copy of the VI Referral Form below.
Individual Education Plan
Children with identified Special Educational Needs (SEN) will have an individual education plan, set up with parents, staff and sometimes health professionals. The plan will be regularly reviewed. This suggested format includes provision for recording information as required by the SEN Code of Practice. Download a copy of the Individual Education Plan below.
Record of Individual Education Plan (IEP) Strategies
This is a system for practitioners who are allocated to do specific IEP work in the classroom. It enables practitioners to briefly record the work to be done each day/week and report on how it went. Download a copy of the Record of Individual Education Plan (IEP) Strategies below.
Special Educational Needs (SEN) Profiles Form
This form provides a record of all the children in the setting who have been identified as having SEN and dates for their Individual Education Plan reviews.
